What can I expect on my first visit?
Upon arrival, you will be asked for your completed paperwork so we can make your chart ready for your evaluation hour. We will need to make a copy of your insurance cards and a picture I.D. at this time as well. Although not needed for your initial evaluation, we will need to have your doctor’s referral for your physical therapy treatment. It is important that you know how to get to here so that you can arrive on time for your administrative appointment prior to your evaluation. Click here for detailed directions.

Insurance Information

Upon request, Center IMT will be pleased to assist you with filing your insurance. As long as we have received all your correct insurance information, claims are electronically filed – usually at the end of the same business day. Since our center is ‘out of network’ for all insurances, the payment should go to you at your home. We are also Medicare providers. As with commercial insurance, we are non-participating with Medicare, so again, the payment should go to you. Medicare will be filed in full accordance with Medicare’s strict policies.

We would also like to offer to help you understand your physical therapy benefits. Please provide us with pertinent insurance information prior to your visit. We will contact your company for information on your specific benefits, and call you back to discuss what you should expect. In addition, we can discuss all options for payment, and decide which will best suit your budget.
